Highlights include keynote speakers at the forefront of health equity in Ontario, breakout sessions ​featuring expert insights on DEI, Indigenous health, sociodemographic data collection, health ​equity strategy, research, reporting and evaluation and a panel discussion.


Emphasizing the need for a collaborative approach to health equity, this conference aims to equip ​participants with the knowledge and resources necessary to drive meaningful change. A virtual ​option is available for the morning session.

Keynote Address

Sane Dube, Director, Health Equity and System Transformation, ​Ontario Health


Sane Dube (she/her) currently works at Ontario Health West. She leads a team implementing ​health equity interventions and supporting system-wide transformation. Her work supports ​communities in defining their own health priorities. Sane is also pursuing a doctorate in Public ​Health (DrPH) at the University of Toronto. Her people are the Ndebele of what is now called ​Zimbabwe. She lives in Treaty 13 territory.

Keynote Address

Fatima Sheikh, Health Equity Specialist, Hamilton Health ​Sciences and Ph.D candidate at McMaster University


Fatima Sheikh (she/her) is a Ph.D. candidate at McMaster University, specializing in the ​intersection of social determinants of health and critical care. In addition to her academic role, ​she serves as a Health Equity Specialist at Hamilton Health Sciences. Her research focuses on ​two key areas: (1) examining how social determinants impact the incidence and outcomes of ​sepsis, and (2) exploring their influence on healthcare delivery to inform policy, promote ​evidence-based decision-making, and foster equitable practices. Fatima's primary interest lies ​in understand how these determinants shape disease patterns, affect patient care, and ​influence healthcare systems and policies.

What is the DEI and Health Equity Knowledge Exchange?



Transforming Care: DEI and Health Equity Knowledge Exchange is an inaugural event dedicated to assisting ​organizations in ​integrating diversity, equity, and inclusion (DEI) and health equity into their operations. The ​conference brings together professionals, experts and advocates from across Ontario to share knowledge, ​discuss challenges and develop strategies for creating a more inclusive and equitable healthcare system.

When and where will the conference take place?


The conference will be held on Thursday, Dec. 5, 2024 at the Holiday Inn Hotel and Conference Centre in ​St. Catharines. For directions, please click here.

Who should attend the conference?


The conference is designed for healthcare leaders, policymakers, researchers, educators and anyone ​interested in advancing DEI initiatives in healthcare.

Will there be a virtual attendance option?


Yes. We are offering a virtual attendance option for those who cannot attend in-person. Virtual attendees ​will have access to a live-stream link broadcasting our opening remarks, keynote address, panel ​discussion and interactive activity. Virtual attendees will also be invited to participate in two virtual ​workshops hosted by Ontario Health West and Scarborough Health Network. Virtual attendees will be ​given access to DEI online training materials.

Will there be opportunities for networking?


Yes. The conference will feature multiple networking opportunities including breakout sessions, ​roundtable discussions and social events. These are great chances to connect with peers and experts in ​the field.
